How To Enroll

Required for All Children:

• Child Enrollment Form (completed)

• Current Immunization Record (Florida Form 680)

• Current Physical Examination Record (within past year)

• Two (2) Emergency Contact Cards

• Fee Schedule (subject to annual increase)

• Parent Agreement Forms (signed)

• Food Program Forms (signed - required for all children)

Additional Requirements for Infants (Under 1 Year):

• Infant Feeding Plan (must be updated as needed)

• Medication Sheet (if applicable)

If Your Child Requires Medication:

• Completed Medication Authorization Form

• Medication in original labeled container

• Written instructions from physician (for prescription medication)

All enrollment forms must be completed and signed before your child's first day. Parents are required to keep all information current at all times, including any changes in address, telephone numbers, emergency contacts, or immunization records.

Tuition Rates & Payment Policies

Registration Fee

$50.00 one-time, non-refundable fee due at enrollment and every year thereafter when children are re-enrolled. This fee reserves your child's spot and is not applicable to tuition.

Payment Schedule & Methods

Tuition for full-time and part-time children is paid weekly in advance. Fees are due prior to service.

Payment Method: Cash only, paid weekly or bi-weekly in advance.

Important: Unpaid accounts will be denied entry into the center. For children using the center on an occasional basis, fees are due when services are rendered.

Late Payment Fees

Late fees apply as follows:

• $10.00 if not paid by mid-day Tuesday

• $30.00 if not paid by mid-day Friday of the 1st week

• $50.00 if not paid by mid-day Friday of the 2nd week (in addition to previous fees)

Multiple Child Discount

When more than one child from the same family attends full-time, a $5.00 tuition discount applies to each additional child. The child with the highest weekly tuition receives full rate; subsequent siblings receive the discount. This discount does not apply to part-time or occasional hourly care.

Important Policies to Know

Enrollment Basis

Granny D's Learning Center enrolls children on a first-come, first-serve basis. Should space become filled, we will start a waiting list and notify parents as soon as space becomes available. To hold a spot on the waiting list, parents are required to pay the $40.00 enrollment fee.

Hours of Operation

Monday-Friday: 6:00 AM - 5:30 PM

Late Pick-Up Fee: Parents are only allowed 10 hours per day for childcare. A late fee of $25.00 per child will be assessed for each 15 minutes after scheduled pick-up time. This policy will be strictly enforced.

Withdrawal Policy

Two-week written notice is required if it is necessary to withdraw your child from the center, or two weeks tuition is due. Withdrawal during a week still requires full tuition payment for that week.

Withdrawal and subsequent re-enrollment requires payment of a $40.00 registration fee.

Vacation & Absence Policy

One week vacation per year can be taken after the child's first year at the center. Parents are required to give two (2) weeks written notice when taking the week of vacation. Otherwise, full tuition will be due for that week.

During the first week of vacation, parents will be responsible to pay half (50%) tuition. The second (2) week will be no charge tuition for their child (children).

Full tuition is due when absences occur due to sickness or any other reasons, legal or religious holidays, or bad weather.

There is no charge for any time taken off by the providers for sick days. However, parents should be prepared with their own back-up in such situations.

Keeping Children Healthy

Immunization Requirements

State law requires that a certificate of immunization be provided to the center at the time of enrollment. A new certificate is required prior to expiration of the current certification.

We require this form of a signed affidavit against such immunizations within thirty (30) days of the child's enrollment.

Physical Examinations

All children enrolled in the program must have a current physical examination on file (within the past year). Health records must be renewed every 6 months for children under age 2, and every two years for those older than two.

Illness Policy

If your child has a fever, sore throat, diarrhea, rash, is vomiting or experiencing any other flu symptoms while at the center, they will be isolated from other children and made comfortable in a quiet area of the room.

We will contact you or your emergency contact to make arrangements for the child to be removed from the center. It is the goal of Granny D's Learning Center to provide a safe and healthy environment for all children.

Children must be fever-free for 24 hours before returning to the center.
